Service - Case Management
CASE MANAGEMENT

Case management is a service provide to persons 60 and older who may be in need of benefits or in home services. Case managers make home visits to evaluate an older person’s need and to assist an older person in gaining access to and receiving needed services. Case managers will coordinate formal and informal sources of support on behalf of the older person.

Service Activities may include:
*Assessment of an older persons eligibility for service programs
*Develop a care plan of services based on an older persons need that will enhance an older person’s independence.
*Provide advocacy on behalf of the older person to access necessary services in the community
*Establish linkages for the older person in the community
*Follow up with services provided
*Monitor’s the older persons continued need for services by making periodic visits to the older persons home and re-evaluating their needs every year.

Case management is a free service provide by the Case Coordination Unit. If you or your loved one would benefit from a visit from a case manager, please call our office in the county in which you reside in.
